Our services and payment information:

 

New Acupuncture Patient Comprehensive Evaluation and Treatment (120 minutes)

The appointment includes a detailed review of your current and past health conditions, pulse diagnosis and other physical evaluations such as tongue or abdominal diagnosis, as necessary to complete a comprehensive treatment plan.  The evaluation will be followed by your initial acupuncture treatment. All acupuncture patients who have had a comprehensive evaluation are given the option to address their health concerns using Chinese herbal medicine as well as acupuncture.

 

 Follow-Up Treatment (45-60 minutes)

This treatment option is for established patients. I will consult with you and conduct shortened diagnostic procedures such as pulse or tongue evaluation to determine treatment progress and make adjustments to your treatment plan as needed. Services rendered during follow-up treatments might include any of the following modalities: acupuncture, moxibustion, cupping, gua sha (press-stroking therapy), application of essential oils, and East Asian-based manual therapies.

 

Payment

Insurance Coverage

I am an in-network provider with Blue Cross Blue Shield, Aetna, and United Healthcare, and will submit claims on your behalf directly to these insurers if your health plan includes acupuncture benefits. I will help you determine what your acupuncture benefits are and whether there are condition or visit limitations. You will be responsible for paying your deductible or co-pay / co-insurance at the time of service.

 

Additionally, if you believe you have acupuncture benefits with an out-of-network insurer, I will provide you with a coded super-bill so that you can seek reimbursement; but I am unable to verify benefits for these plans, so you must do this yourself. You will be responsible for paying the full fee at the time of service, and will receive an out of network reimbursement check from your insurer for their allowable amount.

 

Self-Pay Time of Service Fees

Patients who are not using in-network insurance coverage should expect the following fees for service:

    • New Acupuncture Patient Comprehensive Evaluation and Treatment: $222.50
    • Established Acupuncture Patient Follow-Up Treatment: $95

 

Self-Pay Pre-Paid Discount Packages

For new patients: 

  • Intro to Acupuncture Package: $371.00  (10% discount)
    Includes comprehensive evaluation and treatment, plus two follow-up treatments. Expires six months after purchase date. 

 

For established patients: 

  • Acupuncture 4-Pack: $342 (10% discount)
    Includes four  follow-up treatments. Expires one year after purchase date. 
  • Acupuncture “Frequent Flyer” 8-Pack: $608 (20% discount)
    Includes eight  follow-up treatments. Expires four months after purchase date. 

 

Military service members (both active duty and veterans) are eligible for an additional 5% discount on all packages.

All discount packages must be paid for at or before the first session. All time of service fees, copays, and deductibles must be paid promptly in the office.  I accept cash, Zelle, Mastercard, and Visa

 

Cancellation Policy

All appointment cancellations and re-scheduling must be done with 24 hours notice, except in cases of genuine emergency or illness. Missed appointments or cancellations without proper notice are subject to a late cancelation fee ($70 for new patient appointments and $35 for follow-up visits). These fees cannot be billed to your insurance company.

 

For your convenience, you will receive text and/or email appointment notifications at time of scheduling, and appointment reminders before your scheduled treatment. Please make sure to mark your appointment date and time on your calendar.
